Media Release / Consent Form

For SickFit LLC, White Rhino Coffee, and Women Walking in Waxahachie.

1. Permission Granted

I, the undersigned, grant SickFit LLC and its affiliated brands and partners, including White Rhino Coffee and Women Walking in Waxahachie, permission to use my name, image, likeness, and any statements, testimonials, or content I provide ("Materials") for promotional, marketing, and educational purposes.

2. Scope of Use

This consent includes, without limitation, use of Materials in:

  • Printed collateral and event materials
  • Digital content and websites
  • Social media channels
  • Photo and video productions
  • Advertising and national publicity campaigns

3. Public Distribution and Approval Waiver

I understand these Materials may be distributed publicly, including on national platforms. I waive any right to inspect or approve final edited materials or finished publications.

4. Compensation

I understand and agree that no compensation, royalties, or other payment will be provided for use of these Materials.

5. Age and Authority to Consent

I confirm that I am at least 18 years old, or that consent has been provided by a parent or legal guardian.

6. Assumption of Risk and Release of Liability

Participation in activities may involve risk, including trips, slips, and falls. To the fullest extent permitted by law, participants assume all such risk and release SickFit LLC, White Rhino Coffee, Women Walking in Waxahachie, and their officers, staff, representatives, and partners from liability for injury, loss, or damages arising from participation.

By submitting your separate event form, you grant SickFit LLC and its affiliated partners the right to photograph, audio record, video record, edit, publish, and distribute your participation for media and promotional use under these terms.

Trips, slips, and falls: organizers are not liable for injuries or damages, except where liability cannot be excluded by applicable law.

Full Video and Media Consent Terms

I authorize SickFit LLC and its partners to capture, reproduce, edit, publish, distribute, display, and otherwise use photographs, audio, and video recordings containing my image, likeness, voice, and participation, in whole or in part, in any media now known or later developed, worldwide, on a perpetual, royalty-free basis.

I understand materials may be edited for clarity, format, style, and campaign messaging. I waive claims related to rights of publicity, privacy, and approval of final content, except as limited by applicable law.
